I have updated my PD to 11.2.2 (32651) recently, and the Insert key is not working on my windows 7 guest os anymore.
I have a external USB keyboard with insert key on it. It worked fine on my parallels desktop 11 with windows 7 before, but recently after upgrading my PD, my windows doesn't receive Insert keystroke signal anymore... The Insert key works find when I using remote desktop to other windows 7 on the VMware.

    PD version: 11.2.2 (32651)
    Host: OSX El Capitan
    Guest: Windows 7 64bit

Actually I have the "Insert" key on my external keyboard, and it works fine when I remotely logged onto other Windows VM thru Microsoft Remote Desktop.
It just does not work on the Windows 7 in PD.

Yes. The toggling off, would occur in the OS being used. Look at the _last_ entry in the thread I pointed to. It can be 'on' in everything else.
