Is Duo my only choice now for MFA for on prem Parallels RAS? According to this https://kb.parallels.com/124702 it looks like that feature is no longer available since 2019. Am I understanding this correctly?

works like charm, you only need the azure mfa nps extension:

1.) install nps server
2.) install/execute nps extension powershell script from microsoft: https://docs.microsoft.com/de-de/azure/active-directory/authentication/howto-mfa-nps-extension
3.) configure NPS like you would for terminalserver gateway: https://docs.microsoft.com/de-de/azure/active-directory/authentication/howto-mfa-nps-extension-rdg
4.) configure RAS (Connection, 2 Factor, Provider = Azure MFA (Radius), just set servername and key in youre settings
