eShop Solution Needed

I am new to anything website. Have filled in templates and ready to publish, but I am selling gift packages that the buyer can send to a receipient. HOW DO I set up a Receipient Contact Information (delivery address, phone number etc.) fields in eShop so my customers can include it when purchasing? Also, what about a field if they want to have me attach a note card to the gift that we send out.

As I use in IIS, my site points to one of my application directories.and other applications are virtually inside my site like below:

www.test.com -- point to ---> ../Application Root/app1
www.test.com/app2 -- point to ---> ../Application Root/app2
www.test.com/app3 -- point to ---> ../Application Root/app3
...

What should I do if I want to build the site exactly like this, as you know when I set www.test.com for httpdocs/app1 then I don't have access to other apps that are in httpdocs (../httpdocs/app2,../httpdocs/app3,...) to declare them as virtual directories.

or, if you suggest any way please tell me.
